Kolinski & Crosby, Ltd is an out of network insurance provider. Our practice deals with hundreds of insurance companies on a weekly basis. These insurance companies can sometimes be difficult and time consuming and at times, a patient may be asked to get involved in the claim process. Any changes in your dental insurance should be provided to our insurance coordinator as soon as possible. When filling claims on your behalf, we would like to make you aware of the following:
- As a courtesy, we will file all dental claims on behalf of our patients, however, we are not a preferred provider in any insurance network. Any benefit received for treatment may be processed by the insurance company at a reduced, or “out of network” rate. You will be responsible for the full fees of the treatment provided.
- After 90 days of non-payment from an insurance company, regardless of reason, the patient then becomes responsible for the full billed amount and payment will be requested.
- Our fees are based on the treatment plan as presented by the doctor. The type of treatment you receive from us is based upon our professional judgment. We do not believe it is in the best interests of our patient, or our practice, to compromise our recommended treatment in order to accommodate a dental plan’s benefits.
- Our practice utilizes a specialized x-ray, or CT scan, for the evaluation of the teeth and jaw for implant placement. In many cases, dental insurance does not cover this procedure, however, we still take this x-ray as it allows us to give you the best possible care.
- We will not file claims to medical carriers unless the treatment that is needed is due to an accident or injury. You are welcome to file a claim with your medical carrier and we will provide you with any information that you may need.
- We will submit a treatment plan for pre-approval by the insurance company. However, the financial obligation for dental treatment is between you and this practice, not between the insurance company and this practice.
- We are not licensed to file claims with Medicare. Claims will need to be submitted by the patient.
- The following are some insurance companies/insurance plans who will not pay any benefit out to our practice, as we are not a preferred provider, and all payments will be sent directly to the subscriber. As a result, we will request payment at the time of service.
