<p><strong>Introduction:</strong> Osseodensification preserves bone bulk, facilitates compaction autografting, and deforms trabecular bone in an outward strain, which result in alveolar ridge plastic expansion. The aim of this retrospective study was to evaluate ridge expansion after osseodensification.</p>

<p><strong>Materials and Methods:</strong> Patients treated with implant placement through osseodensification were evaluated. The alveolar ridge width was measured at the level of the crest and 10 mm apical to the crest before and after osseodensification. Insertion torque and implant stability quotient (ISQ) values were recorded at implant placements. Expansion values were grouped into the following 3 groups according to the initial alveolar ridge width: group 1: 3 to 4 mm (n ¼ 9), group 2: 5 to 6 mm (n ¼ 12), and group 3: 7 to 8 mm (n ¼ 7).</p>

<p><strong>Results:</strong> Twenty-one patients who received 28 implants were included. Twenty-six implants were integrated, resulting in a survival rate of 92.8%. There was a significant difference in the mean expansion value at the coronal aspect of the ridge between group 1, group 2, and group 3 (2.83 6 0.66 mm, 1.5 6 0.97 mm, 1.14 6 0.89 mm, P , 0.05). The mean torque and ISQ values were 61.2 6 13.9 Ncm and 77 6 3.74.</p>

<p><strong>Conclusion:</strong> Osseodensification can alter ridge dimensions and allow for ridge expansion. Greater expansion can be expected at the crest in narrow ridges with adequate trabecular bone volume. (Implant Dent 2019;00:1–7)</p>

<h4>Abstract</h4>

<p>Our long-term objective is to devise methods to improve osteotomy site preparation and, in doing so, facilitate implant osseointegration. As a first step in this process, we developed a standardized oral osteotomy model in ovariectomized rats. There were 2 unique features to this model: first, the rats exhibited an osteopenic phenotype, reminiscent of the bone health that has been reported for the average dental implant patient population. Second, osteotomies were produced in healed tooth extraction sites and therefore represented the placement of most implants in patients. Commercially available drills were then used to produce osteotomies in a patient cohort and in the rat model. Molecular, cellular, and histologic analyses demonstrated a close alignment between the responses of human and rodent alveolar bone to osteotomy site preparation. Most notably in both patients and rats, all drilling tools created a zone of dead and dying osteocytes around the osteotomy. In rat tissues, which could be collected at multiple time points after osteotomy, the fate of the dead alveolar bone was followed. Over the course of a week, osteoclast activity was responsible for resorbing the necrotic bone, which in turn stimulated the deposition of a new bone matrix by osteoblasts. Collectively, these analyses support the use of an ovariectomy surgery rat model to gain insights into the response of human bone to osteotomy site preparation. The data also suggest that reducing the zone of osteocyte death will improve osteotomy site viability, leading to faster new bone formation around implants.</p>

<h4>PURPOSE:</h4>

<p>The purpose of this clinical trial was to evaluate the survival rate, bone remodeling, and soft tissue health surrounding variable-thread tapered implants placed in fresh extraction sites and loaded immediately.</p>

<h4>MATERIALS AND METHODS:</h4>

<p>Sixty implants were placed in 55 patients at six centers according to a predetermined protocol. All implants were placed in extraction sockets and were subjected to immediate temporization. Definitive prostheses were placed within the first year. Clinical and radiographic examinations were performed at implant placement and after 3, 6, 12, and 24 months. Assessments of implant stability, Papilla Index, plaque, peri-implant mucosa, and marginal bone levels were performed at the respective visits.</p>

<h4>RESULTS:</h4>

<p>Fifty-five patients were treated and were restored with 58 single crowns and 1 two-unit fixed partial prosthesis. The cumulative survival rate was 98.3% after 2 years. One implant failed prior to 3 months. The mean marginal bone remodeling from implant insertion to 1 year was -0.22 ± 1.30 mm (n = 41), followed by an average bone gain of 0.12 ± 0.77 mm (n = 33) between 12 and 24 months. Mean marginal bone remodeling was -0.10 ± 1.38 mm (n = 35) from implant insertion to 2 years. Papilla size increased significantly over the 2-year study period. Patient assessments of function, esthetics, and self-esteem also showed significant improvement.</p>

<h4>CONCLUSION:</h4>

<p>The 24-month results indicate that the variable-thread tapered implant can be used safely and effectively under demanding conditions as an immediate postextraction tooth replacement.</p>

<p>The purpose of this clinical study is to evaluate the radiographic bone remodeling, survival rate, and soft tissue health surrounding a variable-thread tapered implant immediately placed in extraction sites.</p>

<h4>METHODS:</h4>

<p>Sixty implants were placed in 55 patients at six centers according to a predetermined protocol. All implants were placed in extraction sockets and were subjected to immediate temporization and function. Definitive prostheses (58 single crowns and one two-unit fixed bridge) were placed within the first year. Clinical and radiographic examinations were performed at implant placement and after 3, 6, 12, 24, and 36 months. Measurements of implant stability, papilla index, plaque, peri-implant mucosa, and marginal bone levels were recorded at each visit.</p>

<h4>RESULTS:</h4>

<p>Thirty-five implants were evaluated at both implant insertion and 3-year follow-up. Bone levels were observed at 6 months after surgery and yearly intervals thereafter and remained stable throughout the study. There was a slight decrease in mean bone level from -0.68 mm at implant insertion to -0.93 mm at the 6-month recall and then an increase of bone to -0.53 mm from the reference point at the 2-year follow-up (an average increase of 0.15 mm from implant insertion). Bone levels remained steady between the 2-year recall and the 3-year recall. Papilla scores increased significantly (P &lt;0.001; Wilcoxon signed-rank test) from insertion to the 3-year follow-up, with most of the increase occurring during the first year. Patient assessments of function, esthetics, feel of implant, speech, and self-esteem also showed significant improvement over the course of the study.</p>

<h4>CONCLUSIONS:</h4>

<p>The results, over 36 months, indicate that the variable-thread tapered implant can be used safely and effectively under demanding conditions as an immediate postextraction tooth replacement. Bone remodeling remained stable with a slight increase, and patients expressed high levels of satisfaction with the restorative results over the course of the study.</p>

<p>The long-term impact of private giving and the appreciation of recipients was on display on February 15 when more than 30 alumni, faculty, residents, and staff of the Department of Periodontics joined with Dean Clark Stanford and other College administrators at a special dedication ceremony. A significant major gift from alumni and faculty Dr. Martin L. Kolinski and Dr. Tricia R. Crosby has allowed the College to create a new Postgraduate Periodontics Resident Room named in their honor.</p>
